Why a factory-built firebox has different rules

A manufactured fireplace was tested and listed as one complete assembly: that firebox, those refractory panels, that chimney, that chase top. The clearances it was approved for depend on all of those pieces being the ones the manufacturer specified. That is why we will not put a generic panel or a mismatched pipe into one. It is not fussiness about parts. Substituting components voids the listing the safe clearance to your framing was based on, and the framing behind a zero-clearance unit is genuinely close.

The failures we actually see

Refractory panels crack. They are meant to, in the sense that thermal cycling will eventually do it, and a hairline crack is normally fine. A crack you can fit a coin edge into, or a panel that has crumbled at a corner, is not, because the panel is the barrier between fire and metal. Dampers seize with rust after years of an uncapped flue letting water in. Blower motors on units with a fan die noisily. And on the older units, the chase top above has rusted and rained down the inside of the chimney for years, which is what corroded the damper in the first place.

Masonry fireplaces here, too

Plenty of Kearns houses do have a real masonry firebox. Those wear differently: firebrick joints wash out, the throat and smoke chamber collect the heaviest soot, and the original iron damper warps until it no longer seats. All of that is genuinely repairable, and the masonry shell almost always outlives the parts hung inside it.
